Thank you for submitting your information.  What happens now?

We are open to review charts, reach out to patients,  and call in prescriptions between 7am and 9pm MST, 7 days a week. 

• If you have an urgent situation and you submitted your forms during open hours, we will begin to review your submission. Expect to be contacted shortly through text or a phone call.  In the unlikely event you have not heard from us in 2 hours please text 801-254-0713. 
• If you want to discuss an issue or want a second opinion, expect confirmation in the next 24 hours and a report within 4 business days. 

How to contact us now and in the future?

Our primary mode of communication is through texting.  Texting is easy and all texts go straight your medical chart, which helps serve you better.  We also can coordinate phone calls, video calls, and email. All modes of communication are HIPAA compliant.
